Housing Navigation Services

Housing Navigation connects individuals who are experiencing homelessness or facing eviction from their homes to resources in the community to help either secure or maintain safe and secure housing. The housing navigation team identifies housing placement options, liaises with landlord partners, and provides overall assistance and guidance through the pre-tenancy, lease-up and move-in process.  Housing navigation services are incorporated into all housing programs (temporary and permanent) and new housing navigation services in partnership with the Department of Children and Family Services (DCFS) and Cook County Health (CCH).

Rental Housing Support Program (RHSP)

RHSP provides rental assistance to households with an income of 30 percent or less of the area median income (AMI) and pay no more than 30 percent of their income towards rent.  RHSP subsidies are based on the unit—it is project-based, not tenant-based. That means that when one client vacates the unit, another low-income client moving into the unit receives the same benefits.

Permanent Supportive Housing (PSH)

Permanent Supportive Housing combines rental subsides with voluntary support services to address the needs of adults who have a long-term disability. Program services are designed to build independent living and tenancy skills, and connect people with community-based health care, treatment, and employment services.

Rapid Re-Housing (RRH)

Rapid Re-housing is an intervention designed to help individuals and families that don't need intensive and ongoing supports to quickly exit homelessness and return to permanent housing. Rapid re-housing assistance is time-limited and offered without preconditions — like employment, income, absence of criminal record, or sobriety — and the resources and services provided are tailored to the unique needs of the household.

  • Victory's Path - is a partnership with Hines VA Hospital, we assist military veterans experiencing homelessness transition to stable, long-term housing.  Services include time-limited rental assistance, case management, wrap-around services and life skills to maintain housing independently after completing the program. 
  • Bridge For Youth - serves transition-aged youth (age 18-24) experiencing homelessness or housing instability.  We address the barriers which limit access to housing, such as income, lack of education, background, or discrimination. Tailored services to the young adult population provide a path to independent living through transitional and rapid-re-housing models. Program partners include BEDS Plus, New Moms, and African American Christian Foundation.  Master-leased Transitional Housing and tenant-leased Rapid Re-Housing (TH/RRH) are available.
  • Safe Bridge Housing - Our partnership with Sarah's Inn is a housing program designed for families fleeing domestic violence. The program provides safety planning, court advocacy, case management services, housing navigation and rental assistance for households experiencing domestic violence in west suburban Cook County.  Master-leased Transitional Housing and tenant-leased Rapid Re-Housing (TH/RRH) are available.   • Families in Transition – provides temporary housing for families in a scattered-site setting in communities until permanent housing is identified.

Wraparound Services

The Wraparound Team collaborates with case managers to provide clients with specialized services and a continuity of care across program areas from a client's initial entry and until a permanent destination is found. This interdisciplinary team strengthens the agency's service integration model in the areas of health and behavioral health, employment and income supports, and other critical needs of clients. Professional volunteers from partner organizations supplement the skills of the Wraparound Services Team. 

Behavioral Health Services provide support for mental and behavioral health needs through referrals, counseling services, psychosocial education, and advocacy for clients throughout all Housing Forward programs. This service also provides behavioral health consultation for other program staff in the agency.

Employment Services engages and equips clients in breaking the cycle of unemployment and establishing reliable income. Clients build soft skills, receive career coaching, and job search strategies. Traditional wages and on-the-job skill development are available to eligible clients enrolled in our Workforce Development Program. 

Family Support Services supports the unique needs of families with minor children to guide them in the transition from crisis to housing stability as quickly as possible. A family-centered approach helps them navigate the maze of school, government, and social services which are important steps to strengthening the family's support system. 

Financial Literacy and Budgeting Counseling is provided to current clients and community members to help them gain understanding of how to develop a household budget,  credit history, choosing a financial institution and long-term saving.

Legal Clinic in partnership with Greater Chicago Legal Clinic, we provide pro-bono legal services to support clients in resolving problems such as evictions, criminal records expungement, immigration status, disability benefits, family law, and other issues.  

Medical Clinic empowers clients to take care of their health and well-being and build a personal healthcare routine. Resident physicians from Loyola University's Stritch School of Medicine, our staff community health nurse, and a volunteer nurse perform health assessments, offer health education, and provide basic care weekly at our Interim Housing Program.
