Housing Forward is always working to ensure our services meet the complex needs of the individuals and families we assist. To that goal, we have established a team of specialists and case workers in our brand-new Wraparound Services Program! 

What is the Wraparound Services Program? 

Wraparound Services is our approach to personalized services that will strengthen our ability to support our clients in building long-term stability and improved quality of life. 

Why create the Wraparound Services Program? 

It’s rare that homelessness and housing crisis are solely housing issues exclusively. Most often, they stem from a variety of factors, including mental health, domestic violence, substance use, financial crisis, employment instability/unemployment, and others. Housing alone does not resolve the situation. If the issues that resulted in the housing crisis are left unaddressed, it’s likely they’ll recur and undermine longer-term housing stability.  

Who makes up the team? 

Housing Forward’s Wraparound Services team includes a variety of specialists and case managers who support clients in addressing a variety of challenges and issues.  

• Behavioral Health Specialists: provide support for mental and behavioral health needs 
• Community Health Nurse: provides direct care, referrals, and resources for clients’ primary healthcare needs 
• Community Case Managers: focus on long-term needs of those transitioning to more independent housing 
• SOAR Case Managers: provide case management for those who are using social security disability benefits 
• Family Support Specialists: provide support for parents and families exiting homelessness 
• Employment Specialists: provide employment readiness and career passport services 
• Client Assessor: provides centralized client intake at Housing Forward 
• Data Specialist: collates & analyzes program data
